I do similar to Toogie - different feeling fabrics front and back - generally something bumpy and something smooth. I do make mine about 12x12 too.
If you do an online search for tag blankets, many tutorials will come up, however, be aware that these blankets are patented. The company is very aggressive in defending this patent. Do not try to make these and sell them even if you use a different name for them.

The children like the texture of the different fabrics. On mine I made before I used the bumpy minky on one side and do whatever you want on the other with a different texture, whether it is all cotton or a mix of cotton and minky. If I know the baby's name I emb it on the Minky like Caroline said before I sew the pieces together. Nora was the one most attached to hers. She like the feel of the ribbons too. Ribbons were sheer, grosgrain and satin, but she loved the satin best. I just measured one I have cut and it's 14 inches square. However, if 12 inches is all you have, I think it would be fine. Is this for a new baby in the family?

I've never made one and want to experiment before I need to make one for someone.
I use cuddle fleece, sometimes on both sides or s pretty printed cotton fabric on the reverse. I embroider the childs name on the cuddle fleece. With the ribbon I personally use a selection of different textures. Fun to make and babies and their mums love them
