I stitched the design on an offcut of dupion silk (any fabric will work, but silks and satins are nice) Then fold a piece of card and cut out a shape that suits the design and glue it in behind. (I used to freehand the cutout with a craft knife, but now I have a coluzzle and fiskars templates and cutters) Then cut another piece of card and glue it over the back to hide the back of the stitching. Sometimes I also cut a piece of thin batting in the shape of the cutout on the card and put it behind the fabric - just makes it look a bit fuller.

Oh and I use a punch for the corner patterns, and those silver or gold sticker words and shapes.
