I have many times... slow the machine down, use a leather needle and stitch away. Make sure your design isn't too dense or you'll have trouble. Embroidery Library actually has designs digitized specifically for leather.
I went to an Anita Goodesign conference and they suggested embroidering onto fabric and then sewing onto the leather item.

belle, 1st - let me ask - how are you hooping? What thickness is your leather? Are you using a leather needle? I have embroidered on leather pillows, purses and many many picture mats without any pulling away from the embroidery. It must really be dis-hearting to have this happen. How can we help? Where did you get your design digitized 2 times?
