I just finished an outfit with monogram letters, and what I did was the first name is the first letter, the big-center letter is the last name and the third letter is the middle name of the person or child.
I would also go along with what Melita says. I think that is the usual format.
Wow... I have a mink jacket that has been passed down to me that has initials of my GGG grand mother...on the inside left... they are as follows C J C
Her name is Christina Josephine Callaway..... The J in the middle is the largest of the letters.....

Thanks Jo, you're up late too! I need to design some pillow cases for one of my patients. Her son is getting married and I just wanted to design a monogramed pillowcase for her.
Don't know if my info helped or hindered !!.. the stoll I have is very old.. I often wondered why the middle initial was larger than the others !!! have not gotten an answer as of today!!! lol.
From the answers I am getting the last name should be in the middle, this is why I posted this question because I have seen mixed answers. I am going to go with the most common suggestion. Thanks for your input. Maybe they did monogramming different back then.
Usually the first letter is the first name, the center letter is larger and is the last name, and the last letter is the middle initial. Hope this helps.
First name initial, surname initial, middle name initial.
or husband's first Initial, surname initial, wife's first initial.
This is the standard as far as I am aware.
