Let's take an example. Capital T large with tessa across.I put in the large T first. Then put 'tes number of spaces ta' (or divide the name in two parts -makes tessa a bad example as it has 5 letters-, put in 'tes' and 'sa' separately) and place them the way I want.
I have done these with Artista Software and with it, I put the Large beginning initial down first and then the name across it in a place that doesn't distort the beginning letter and then I removed overlapping stitches...well, actually I click on an icon and the software removed overlapping stitches.;)
it is where the first letter of your name is big centered and then your name across it

MM, I think that you put the LARGE letters first - then you put the SMALL letter on top of them. Hehehe! *4U
