Pacifier case and bottle thermos bag for one of my baskets. The thermos bag is lined with bright lite to keep the cold or heat. The designs are from stitch delight,

I made the pattern up myself, sorry I can't be of help, I do not know where you could find one, I didn't even look, just decided to wing it, turned out pretty good,
