I just remembered a site that has 4x4 dolls and are about $6 at link below-tho I don't care for the faces-but doll and outfits are cute.
They look like they might work but I saw one in projects that I really like...going to ask her what size it is if I can fig out how to send a PM.
I have sent her a message to see if these are ITH. I like the little red riding hood.

The ones I have are for 5x7 hoops. However, there is a site Sewing for Sarah that has tons of In the hoop projects for children and lots are used for these purposes. Maybe she has something for a reasonable price. Her site is hard to navigate so look on the left side and go down to Softies 1 and 2
areas. Also, ITH projects 1 &2

BTW those little dolls are so cute. I made
a tiny one but lost it during a computer crash- not exactly 4x4 but I don't see why you could not separate the head and torso and legs to fit in 4x4.
If you sign in to Sewforum there is a doll and bedding is also available.

The Kimmy Doll on this site. This link is for the mini doll but she has other sizes. You're a wonderful women to do this for the hospice children. Great idea.
I relooked at the site and not sure that this will fit your 4x4 hoop. This mini says for a 5x7.
