You can make the blanket any size from 12" square to a full size blanket. No rules, just what you are trying to achieve! I make full size blankets for my DG's and they loved them, but the 3rd request from DD was for a small 12" one, to drag around as a "lovey". Still sleeps with it at age 8! The loops are just about 4" as stated previously, folded in half and inserted into the hem or seam. I placed them on the square with the loop to the inside, stitched around and folded the hem over, stitched again and folded each one out to face the outside (hope that's clear) to reinforce the stitching as they get tugged on ALOT! Very cute.

Just put my grand baby's name on one! Seems like the tags were all ribbons or different kinds & they were folding into a loop with raw edges inserted into the side seams all around. I THINK the folded piece was about 1 1/2 inches. Maybe cut the ribbons 4" in order to have seam allowance.
What is a taggie? Ties?
I tried to describe and could not! There is a sample of one at Target
