I need to find labels or patches that are about 4-3/8" long and 1-1/4" wide. I need to embroider on them for a non profit organization. Thanks for you help.

I have been doing embroidery for a few years now. But every once in a while the stitches come undone from the design. How do I correct this problem? It seems to happen on shirts mostly.
