I always the babys name..after all once you have a baby you become " so and so mom" :-)

I'd do the baby's name, too, as Emily suggested. Mom will probably figure out something cute to do with the tag, maybe as a decoration in the baby's room. Seems funny that in these days you can go to a baby shower and not only know if it's a boy or girl, but the baby's name as well, hee hee hee. In my day, it was a big surprise! I prefer the baby's name. and I suppose the mother will love it to see the name of her new born added!1

If You know FOR SURE the name of the baby then I would say to use it!!!

Thank you all for your comments. I am going to put the Baby's name on the tag. I too think the Mother would like it but I just didn't want to do something not appropriate.
