More than likely it will be passed on to a family member so I would put the name relevant to the person most likely to receive it. Mom or Granny would be more personal than your name.

When I do a quilt for "Me" I put my name on it. When I do for say one of the grandkids, I write grandma and below that my name. that way, they won't ever forget what grandma made it. Don't forget to add the date.

well it's up to you but i would write my name and the date with it.You said the quilt is for yourself., and if in future the quilt goes perhaps to the grandchildren, perhaps they have another grannie too. with your name on it there cannot be a misunderstanding who made it.

I would put my name and the date and a short message to anyone that it could be handed down to at a later time.

Name the quilt
The Rose Family
by Granny
Yourname
date
This way a 100 years from now someone will be able to trace it along with its history.

When I make for my grandson I call myself Grandma Sally as he has 2 grandma's and always the date.. With your quilt I would put as much detail as possible especially if you are hoping this to become an heirloom... Much like what kttyhwk4 suggests...Hugs Sally
