I think it must be a "picture" or design of a rattle, not a real one. Watch out for those vortexes, I hear they are almost impossible to get out of. ;)

Considering the little elephant is in the middle of a bunch of baby stuff (THANKS, CUTEEMBROIDERY! My babies are going to be decorated in lots of CUTE stuff now!) I figured he was a picture of a baby toy, like a rattle or a teether, and the hole was to form his handle.
