I think it is intended to be used as a picture frame. Trim the photo to size to slip into the pocket of the frame,when it needs to be washed slide the photo out.

I've made it as a picture frame as Bev suggested. You also could print your photograph onto fabric with a T-shirt transfer or with the bubble jet system and then include it at the appropriate point in the design but you would have to get the picture exactly the right size.
