This never happened to me before, I used a new embroidery needle, and a tear away stabilizer the fabric is 97% cotton and 3% lycra. My Machine is Brother se-400, do you have any suggestions? I'm starting X-mas gifts and I have several western shirts to sew and embroider and I want it to be prefect hence the test patch. Thanks. PS The design is very dense too.

I finally reduced the tension and the second stitch out worked better, I hope this solves my problem so when the real deal is stitched I don't mess up the project.
