I made this for my daughters friend, who son is autistic.

It is embroiderydesigns.com. In search bar, type in Puzzle Ribbon. I added the eyelet to the top to make it a charm. The eyelet is under DBC, Shirlene posted it awhile ago. Hope this helps you find it.
