I have found that it is usually the stabilizer used - that is if the design was digitized properly. If you want to fix it, you could set a narrow satin stitch on your machine NOT in embroidery mode and go around the outline just covering the part it doesn't match up. ;)

Use more stabelizer. I didn't like to use more than one piece either. I felt like I was wasting it or it shouldn't be necessary, but it is. The way I have gotten around this and feel better about it is this. I put one piece of stabelizer in the hoop with my fabric, then I use left over pieces of stabelizer I have previously used and torn away from other projects. I save the larger pieces instead of throwing them away. I use these to sort of make a patch work undeer the hoop. I do not piece it together or hoop it I simply lay it in place under the hoop before I start stitching. The stitches hold it in place and it works as extra support for the stabelizer that is hooped.
